a) An application form for a copy of bóng đá hôm nay trực tiếp diploma or certificate, providing information about bóng đá hôm nay trực tiếp diploma or certificate already issued;
b) bóng đá hôm nay trực tiếp original or a certified copy of bóng đá hôm nay trực tiếp ID card, citizen's identity card, or valid passport for bóng đá hôm nay trực tiếp recipient to check;
c) In case bóng đá hôm nay trực tiếp requester for a copy of bóng đá hôm nay trực tiếp diploma or certificate from bóng đá hôm nay trực tiếp master register is specified in Clauses 2, 3 of Article 30 of this Regulation, a letter of authorization (for authorized persons) or documents proving bóng đá hôm nay trực tiếp relationship with bóng đá hôm nay trực tiếp person granted bóng đá hôm nay trực tiếp original diploma or certificate must be presented;
